It's the combination of "Often Evil tribal barbarians" with "breeds with almost anything" that leads readers to presume rape. Many pre-3e books left it as more than just implication.

4e and 5e moved away from it a bit - emphasising that orcs only breed with nonorcs when they think it will strengthen the tribe - and suggested that this might involve deals between orcs and powerful humans that the orcs respect.

I don't get why Kyrie was so upset, I mean V did ask to make sure their kids' injuries (just broken legs, after all) weren't life-threatening -- and even pulled the stakes out -- before reanimating the dead dragon so she could witness the mass murder of her extended family tree.
