Its a really easy conversion (Fort takes Str and Con, Ref takes Dex, Will takes Int, Wis and Cha), you just need to make a few decisions.
Does each save have one contributing stat, choice of two or total of two?
Do you want each class to be proficient in one save type, two types or proficiency in one and half-proficient in a second?Roll for it 5e Houserules and Homebrew

Fortitude - Con/Str
Reflexes - Dex/Int
Will - Wisdom/cha
Reflex can be mental quickness or honed reflexes.

If one was going to go with the three saves model, I like the way Kryx does it the best.
Fortitude - Constitution
Reflex - Strength/Dexterity
Will - Intelligence/Wisdom/Charisma

4e's saves were one of the things it did well.
I am also fond of 5e's; it accomplishes the goal of making vulnerabilities present is all monsters and builds, and complements bounded accuracy well.
I know we get periodic complaints about bounded accuracy, but I think it does the job of allowing more things to be threatening at least in the short term and also giving even lower-level PCs a fighting chance in a lot of cases. It can also be frustrating for PCs who want to be untouchable, but 5e is very against the idea of being truly untouchable. Conversely, though, the reason CR calculations are based around hp and damage primarily is because, if you keep those in the right range, even a seemingly unstoppable monster that hammers weak saves is surprisingly survivable in 5e.
